- Please contact Mrs. Jeanne Hablitz for all Availability (732-872-1476 ext. 11).
- Completely fill out Usage Application. Please include proof of insurance.
- Return all forms to Mrs. Jeanne Hablitz via us mail or e-mail @ [email protected]
- Your application and dates requested must be submitted prior to the monthly BOE Meeting. If submissions are late, it may be declined.
- If approved, you will receive notice via e-mail (please supply valid e-mail address).
